  • The General Safety Orientation allows you to obtain access to the systems in the AMRIS facility (with the exception of the 11T).  Please use the form below to request a General Safety Orientation. The Safety Coordinator will respond with the next available dates for the class. Prior to attending class, please do the following:

     Read the AMRIS Safety Handout

     Register with the NHMFL User Program

  • After-hours access to the MBI will require your Principal Investigator’s and departmental Administrative Assistant’s signatures on the back of the form at the top. The Administrative Assistant will typically be the Security person in your department who issues your lab keys.

  • When you have completed the Safety Training and you have a completed and signed green access form, bring the form to our Office Manager, Angie Fuhr, in room LG-115.

  • You will be contacted by email by our Office Manager to pick up your key from room LG-115 when it is ready.

  • 11T Access – Users requesting access to the 11t system will first need to complete the General Safety Orientation and then register for the 11T Safety Orientation. Contact the Safety Coordinator for registration information.

Request for Safety Orientation

  • Please enter the dates and times you are available so that the Coordinator may choose a time suitable for the group.
    Access to the 11T requires prior experience with the 4.7T and completion of an advanced safety orientation. The Safety Coordinator will provide you with further instructions after completion of the General Orientation.
